Attach a PMS "yellow" sticker to your completed application and send the form directly to the Lender. Please click here to download a sticker.
For Online Applications
Ensure that you select the "PMS" payment route on the lenders website application from their dropdown menu or complete the appropriate section on the form
If PMS is not available as an option you may need to contact the lender to register with them to submit business.
